When you connect to WhatsApp Web on your computer, the details of the connection are recorded on your WhatsApp app. If you notice a connection that is not familiar to you, then more likely than not, someone is monitoring your account using a WhatsApp Web session.

For another person to activate your WhatsApp account on another device, you have to be logged out of the current device. If you receive a message that WhatsApp has sent you an activation code, and you are not aware of it, then more likely than not, someone else is trying to gain access to your WhatsApp account.
